01 · The problem

Where they slip away.

  1. 01 / 04After hours

The phone rings out at 6:01 pm.

Plenty of patients look for a doctor after work, when the office is closed. A voicemail and a contact form are often all they find.

  2. 02 / 04Search answers first

68.01% of searches ended without a click.

That's US Google searches, January to April 2026, that never reached a website. Patients choose from the answer on the page, so your facts have to be in it.

    Source: SparkToro, 2026 zero-click study
  3. 03 / 04The front desk is buried

    The same questions. All day.

New patients? Which insurance? How do I pay? I can't log in to the portal. Every one is a phone call your team takes instead of looking after the patient at the counter.

  4. 04 / 04Patients drift

    One visit, then silence.

Without reminders, recalls and a reason to come back, patients who liked you forget to rebook, and nobody asks them for the review they'd happily write.

Engineering notesHow the patient lifecycle works in a medical practice engine

Most practices lose patients in the gaps: the call that came in after closing, the new-patient question nobody answered until the next morning, the reminder that never went out, the recall nobody had time to make. A web engine closes those gaps by connecting the practice website to the systems that handle the conversation after the click.

When a call is missed, the practice's CRM sends an automatic text back, with an opt-out in the first message, pointing the patient to the website. There, Engine Agent introduces itself as an automated assistant, answers practical questions from information the practice approves (new patients, insurance plans listed, hours, forms, the patient portal and how to pay a bill) and logs each conversation to the CRM with a summary and a next step. It never gives medical advice, asks patients to keep health details out of chat, and passes every clinical question to the care team. Clinical history stays in the practice's own record systems.

Booking happens in the chat when the practice's calendar is connected, or through its online booking. Reminders, confirmations and yearly recalls go out through Dash Funnels, the CRM behind every web engine, as workflows our team sets up. The CRM record remembers each patient's visits, preferred times and how they like to be reached, so those messages can be personal. Texts go only to people who have agreed to receive them, and every message carries an opt-out. After each visit Bird Local sends a review request to every patient the same way. Reviews are never filtered or gated, and the team replies from one place.

Karim and the conversation above are an illustration. The exact messages, timing, booking path and workflows are configured for each practice during setup, around the practice-management and record systems it already uses.

Engineering notesWhat separates a practice website that fills the schedule from one that just exists

Most practice websites are built like brochures: a welcome message, a photo of the building, a staff list and a contact page. They describe the practice. The patient, meanwhile, arrived with a question (are you taking new patients, do you take my insurance, can I book tonight) and decides in seconds whether this practice can answer it.

A web engine starts from that patient. The headline answers in patient language, booking, calling and texting are always one tap away, forms are online, and the insurance plans the practice accepts sit on their own page in plain words. Accessibility is part of the same work: readable type sizes, strong contrast, large tap targets, visible keyboard focus, real headings and labels, and one accessibility menu for text size, contrast and motion.

The same structure is what search engines and AI assistants read. The practice, its providers, services, hours and location are described in schema.org structured data, the questions patients ask are answered in the first paragraph of their own pages, and every fact matches the practice's listings and Google Business Profile. Light, fast pages served from the edge make all of it useful on a phone straight away.

“Typical” describes the patterns we find most often when we audit practice websites; individual sites vary. PageSpeed is measured with Google's PageSpeed Insights, and results vary by page and content.

06 · Questions

Questions people ask us.

Does Engine Agent give medical advice?

No. Engine Agent introduces itself as an automated assistant, answers practical questions from information your practice approves (new patients, insurance plans you list, hours, forms, the patient portal and paying a bill) and hands every clinical question to your team. It asks patients to keep health details out of chat, and is set up to tell anyone describing an emergency to call 911.

What does Engine Agent collect from patients?

What a front desk needs to follow up: name, contact details, the kind of visit, preferred times and, if you want it, the insurance plan to verify. Clinical history stays in your own record systems. Patients choose whether they're happy to get texts from the practice, and every text carries an opt-out.

Does it work with our EHR or practice-management system?

It works alongside it. Your EHR or practice-management system stays the patient record; the engine handles the website, the first conversation, follow-up and reviews around it. Whether bookings flow straight into your system depends on the system and its booking options, so we check yours during discovery and set up the cleanest path: online booking, a booking link you already use, or a request your team confirms.

Will patients know they're talking to an automated assistant?

Yes. It says so at the start of every conversation, and a patient can ask for a person at any point. During the hours you set, the conversation goes to your team with the history attached, so nobody has to repeat themselves; outside them, the patient is pointed to the right contact.

How do review requests work?

After each visit, Bird Local sends a review request to every patient, never only the happy ones, because selectively asking satisfied patients goes against Google's review policies. Your team replies from one place, and we help keep replies general so they don't confirm that someone is a patient.

How many pages will our website have?

It depends on your services, the conditions you treat and the areas you serve. Across nine live VIS Mountain builds the average launch was 94 pages: a page for each service, the questions patients ask about it, and each area you cover.

Can you get ChatGPT or Google to recommend our practice?

Nobody can honestly guarantee that. What we do is make your practice the clearest, most consistent answer for your services and area (structured data for the practice and its providers, answer-first pages, matching listings and genuine reviews). On Growth plans, we then check month over month whether the major assistants are naming you.

How quickly can we change things, like a new provider, new hours or a holiday closure?

Send the request through the Hub at any time. Most changes are live within 24 hours, and larger projects, such as a new section of pages, are scoped with you first.
